Sports massage appointments are to help release and reduce tension in your muscles caused by sports and the stresses of daily life. 
A medical form will be completed online for your first booking. 

Once form is filled out, I will contact you via phone call (if needed) to discuss and then we can go ahead with the appointment as planned.

All other information will be discussed in the assessment.
